Adult Membership

You have the choice of applying online or downloading a form. All you need is proof of identity.

• To make your application easy, ensure you have access to several identification documents, e.g. Passport, Drivers Licence, Medicare card.

Please note:

• Adult members need to purchase a $2 share
• If you are not an Australian citizen or resident, download, print and complete the Application form. Do not apply online.
• If you need to stop and save your online application, you can retrieve it at any time. Retrieve a saved Adult Membership application

Under 18 Membership

You have the choice of applying online or downloading a form. All you need is proof of identity.

• A copy of the Birth Certificate is required for identification

Please note:

• For additional Account Signatories on an Under 18 membership, please complete the Account Signatory form. Alternatively, the parent/guardian may choose to become an Adult Member at First Option.
• If you need to stop and save your online application, you can retrieve it at any time. Retrieve a saved Under 18 application

Business Membership

Business Memberships are available to those who have an affinity with First Option Bank. This includes existing First Option members, their immediate family and those employed by a First Option host company.

Apply for a Business Membership if you have a:

• Company
• Partnership
• Formal Trust
• Superannuation Fund
• Unincorporated Associated

Download, print and complete the Business Application form and forward it to First Option.
Please note:

• Business members need to purchase a $2 share
• Sole Traders should print and complete the New Savings Account form

Become a Signatory to an Account

As a Signatory, you can carry out withdrawals on specific accounts for any purpose, including signing cheques.

You can also make enquiries about account balances and transactions on specific accounts, including any debit balance or available credit on a transactional account.

Simply download the form and complete the sections that relate to your needs.
